Thyroid Disease in Pregnancy - A Guide to Clinical Management

Abstract

Thyroid nodules and cancer are more commonly encountered in women of reproductive age. Its diagnosis during pregnancy is challenging for the clinician. The overall prevalence of thyroid nodules and cancer during pregnancy is 3–21% and 14.4 per 100,000 births, respectively. All palpable nodules detected during pregnancy must be evaluated by ultrasound imaging of the thyroid and neck. Benign nodules can be under surveillance during antenatal period. Suspicious nodules should be subjected for tissue diagnosis to rule out underlying malignancy. A multidisciplinary approach is to be instituted following the diagnosis of malignancy for optimal obstetric and oncological outcomes. Clinicians must be aware of management guidelines for thyroid malignancy during pregnancy and postpartum period based on various histological types. In cases of multiple endocrine neoplasia, genetic counselling should be offered to the couples.
