Zusammenfassung
Die Ultraschall-gesteuerte perkutane Nierenbiopsie stellt den Goldstandard für die Diagnostik der meisten primären und sekundären renalen Erkrankungen dar. Ohne Nierenbiopsie ließen sich viele renale Pathologien und seltene Systemerkrankungen nicht diagnostizieren; zudem stellt sie einen unverzichtbaren Bestandteil in der Nachsorge der Nierentransplantation dar. Generell geht die Ultraschall-gesteuerte perkutane Nierenbiopsie mit einem geringen Komplikationsrisiko einher. Die häufigsten Komplikationen umfassen Blutungen oder arterio-venöse Fisteln. In der Hand des Geübten lässt sich die Prozedur jedoch ohne größere Risiken durchführen. Die Prostatabiopsie erfolgt leitlinienkonform meist transrektal mit 10–12 Stanzzylindern. Trotz allgemeinem Konsens einer perioperativen Antibiotikaprophylaxe ereignen sich in 3 % lokale und systemische Infektionen (Prostatitis bis zur lebensbedrohlichen Sepsis), vor allem bedingt durch eine zunehmende Resistenzentwicklung der Uropathogene. Der perioperativen Antibiotikaprophylaxe wird ein sehr hoher Stellenwert beigemessen. Eine zielgerichtete Prophylaxe nach Rektalabstrich ist dabei eine gute Option, auch ist eine augmentierte (kombinierte) Antibiotika-Prophylaxe möglich. Nötig ist eine Antibiotika-Prophylaxe mit insgesamt 24-stündiger Wirkdauer. Die Stanzbiopsie über das Perineum ist eine das Infektionsrisiko reduzierende Alternative. Zunehmend kommt die MRT-unterstützte Fusionsbiopsie mit höherer Trefferquote, aber auch höherem Aufwand zum Einsatz. Die zudem auftretenden Komplikationen, wie Blutungen, Harnretention, lokale Beschwerden sowie Erektionsstörungen sind zumeist selbstlimitierend. Unter 1 % der davon betroffenen Patienten bedarf der stationären Therapie. Trotz großer Fortschritte in der Bildgebung können manche Raumforderungen nicht eindeutig zugeordnet werden. In diesen Fällen ist die bildgebend-gesteuerte Biopsie indiziert. Je nach betroffenem Organ und Lage der Raumforderung (z. B. Lymphknoten, Leber, Nebenniere, Pankreas, Lunge, Knochen) bieten sich Sonographie, CT oder MRT zur Bildgebung an. Nach der Biopsie-Technik unterscheidet man die Feinnadelaspirationsbiopsie und Stanzbiopsie, wobei letztere den Vorteil hat, die Gewebearchitektur zu erhalten und damit eine genauere histopathologische Diagnostik zu ermöglichen. Die bildgesteuerte Biopsie sollte nach komplettem Staging nur dann erfolgen, wenn sich daraus therapeutische Konsequenzen ergeben. In der Regel sind Komplikationen selten und meistens konservativ zu beherrschen. Da die meisten Komplikationen innerhalb der ersten 4 bis 6 Stunden auftreten, sollten in dieser Zeit engmaschige Kontrollen erfolgen. Die urodynamische Untersuchung ist ein invasives Verfahren mit dem Ziel, Symptome eines Patienten unter Messbedingungen zu reproduzieren, diese zu objektivieren, die Urzache zu identifizieren und die zugrunde liegenden Funktionsstörungen zu definieren. Komplikationen sind relativ selten, können aber dennoch auftreten. Das Spektrum der Komplikationen reicht von Katheter-assoziierten Problemen mit Dysurie, Schmerzen oder Blutungen bis hin zu Harnwegsinfektionen oder Miktionsstörungen. Eine entsprechende Therapie ist abhängig vom Schweregrad der jeweiligen Komplikation. Diese kann von einer Erhöhung der oralen Flüssigkeitsaufnahme bis hin zur antibiotischen Therapie oder einem Katheterismus der Harnblase reichen.
