Zusammenfassung
Zielsetzung von Antiseptic Stewardship (ASS) ist die Eindämmung der Selektion Antibiotika-resistenter Bakterien und Pilze durch Einsatz von Antiseptika anstatt von Antibiotika, sofern durch Antiseptika eine vergleichbare oder höhere Effektivität zu erwarten ist. Die Notwendigkeit für ASS ist gegeben, da topische Antibiotika zu den meistverschriebenen Antimikrobika gehören, obwohl die Evidenz nur für wenige Indikationen nachgewiesen ist. Bei superfiziellen Augeninfektionen werden in Deutschland überwiegend Antibiotika, die sowohl zur lokalen als auch zur systemischen Anwendung vorgesehen sind, lokal angewendet, obwohl auch am Auge die Anwendung von Chemotherapeutika wegen des Resistenzdrucks auf Bakterien abgelehnt wird, da sich der generelle Erregerwandel auch im Erregerspektrum okulärer Infektionen sowohl bei Endophthalmitis als auch bei bakteriellen Isolaten vom Auge widerspiegelt (Gentile et al. 2014; Wang et al. 2015a). Hinzu kommt, dass okuläre Infektionen durch multiresistente Bakterien deutlich zunehmen (Rameshkumar et al. 2016), verbunden mit Therapieversagen, und Carbapenemasebildner bei Neonaten nosokomiale Ausbrüchen mit Konjunktivitis verursacht haben (McGrath et al. 2011). Allerdings ist nicht nur die Studienlage zum Einsatz von Antiseptika unbefriedigend, sondern in Deutschland stehen derzeit nur NRF-Rezepturen mit PHMB (0,02 und 0,04 %) und PVP-I (1,25; 2,5 und 5 %) für die Augenantiseptik zur Verfügung. Als antiseptisches Fertigpräparat stehen in der Schweiz und in Frankreich Augentropfen auf Basis von Hexamidindiisethionat mit der Indikation Blepharitis, Konjunktivitis, Keratokonjunktivitis und Dakryocystitis zur Verfügung. Dagegen ist der in Augensalbe enthaltene Wirkstoff Bibrocathol antiseptisch unwirksam.
