Zusammenfassung
Aus der Literatur ist eine hohe Inzidenz frontobasaler Mitverletzungen bei schweren Mittelgesichtsfrakturen bekannt. So konnte bei 4 % der hohen Oberkieferfrakturen eine begleitende Basisfraktur mit Liquorrhoe nachgewiesen werden. Diese Komplikation ergibt sich nicht nur wegen der engen topografischen Beziehung der knöchernen Strukturen, sondern auch wegen ihrer pathophysiologischen Verflechtung. Die Trajektorien des Mittelgesichts strahlen in die Schädelbasis ein; die feste Anheftung der Dura an den grazilen knöchernen Leisten begünstigt die Bildung von Liquorfisteln. Insbesondere Kräfte mit vertikalen Komponenten führen zu frontobasalen Verletzungen.
