Zusammenfassung
Die arthroskopisch unterstützte Arthrodese des oberen Sprunggelenkes ist ein etabliertes Verfahren. Sie ist für den arthroskopisch versierten Operateur sicher und reproduzierbar durchführbar. Es wird eine Technik vorgestellt, bei der über zwei vordere und einen posterolateralen Arthroskopie-Zugang eine vollständige Entknorpelung und knöcherne Anfrischung der Gelenkflächen ohne instrumentelle Distraktion erreicht wird. Zwei von posterolateral und posteromedial perkutan eingebrachte Großfragment-Schrauben stabilisieren die Arthrodese. Fakultativ kann eine Spongiosaplastik hinzugefügt werden. Die Ergebnisse zeigen in der Literatur eine Überlegenheit in der Fusionszeit im Vergleich zum offenen Vorgehen. Die Technik ist jedoch nur bei degenerativen Sprunggelenken ohne große Fehlstellungen möglich.
