Zusammenfassung
Die proximale spinale Muskelatrophie (SMA) ist eine genetisch determinierte Erkrankung mit autosomal-rezessivem Erbgang und Manifestation von der Neugeborenenphase bis in das Erwachsenenalter. Sie ist charakterisiert durch eine progressive Denervation der Skelettmuskulatur, im Wesentlichen verursacht durch den Verlust der alpha-Motoneurone im Vorderhorn des Rückenmarks. Die resultierende klinische Symptomatik ist gekennzeichnet durch eine Hypotonie, eine meist proximale symmetrische Muskelschwäche und Muskelatrophie sowie Areflexie. Neben Ätiologie, Pathogenese, Klinik und Therapie umfasst das Kapitel auch die Darstellung der Subformen, wie der autosomal-rezessiven spinalen Muskelatrophien Typ I, II und III, sowie Fallbeispiele aus der Praxis.
