Zusammenfassung
Mit der Entwicklung und häufigen Anwendung moderner Reanimationstechniken hat sich auch die Zahl jener Patienten erhöht, die nach zerebraler Hypoxie/Ischämie ein schweres neurologisches Defizit entwickeln. Am häufigsten tritt dabei eine diffuse Ischämie oder Hypoxie bei Herzstillstand, synkopale Episoden oder pulmonalen Erkrankungen auf. Die klinische Erfahrung zeigt, daß selbst kurz anhaltende anoxisch-ischämische Perioden, die weniger als 2 min dauern, bleibende Hirnschäden hervorrufen können. Üblicherweise ist es sehr schwierig exakte Angaben über die Dauer einer hypoxischen Episode zu erhalten. Wenn die Sauerstoffversorgung des Gewebes rasch wiederhergestellt werden kann, kehrt das Bewußtsein innerhalb von Sekunden oder Minuten zurück, wenn aber der Sauerstoffmangel länger als 1–2 min andauert, können sich ein komatöser Zustand, ein Sopor oder ein Verwirrtheitszustand entwickeln, die über Tage anhalten oder aber als Dauerfolge bestehen bleiben.
