Zusammenfassung
Anhand eines geeigneten, gut reproduzierbaren Endotoxinschockmodells der Ratte (LD 80) wurde in einer randomisierten, kontrollierten Studie im ersten Teil der Untersuchungen die Effizienz dreier verschiedener Glucocorticoide (Methylprednisolon (MP), Dexametason (DEX), Triamcinolon (TRI)) in unterschiedlichen Dosierungen auf die Parameter Überlebenszeit und Langzeitüberlebensrate (Beobachtungszeitraum 96 h) geprüft. Für alle gleich wurde dosisabhängig sowohl eine Verlängerung der Øberlebenszeit (ÜLZ) als auch eine Erhöhung der Überlebensrate (ÜLR) beobachtet (identischer Wirkungsmechanismus!). In den jeweils effektivsten Dosierungen zeigten sich keine Unterschiede zwischen MP 50 und 100 mg/kg KG, DEX 8 mg/kg KG und TRI, 10 mg/kg KG (ØLR 90 – 100%). Höhere Dosierungen (MP 200, DEX 16, TRI 20 mg/kg KG) führten jedoch in allen Fällen wieder zu starkem Abfall der therapeutischen Wirksamkeit. Dieser nicht auf Eigentoxizität zurückzuführende unspezifische Effekt könnte mit einer Risikoerhöhung für das Auftreten unerwünschter Nebenreaktionen verbunden sein.
Inwieweit eine Hemmung der Induktion des histaminbildenden Enzyms Histidindecarboxylase (HDC) in allen Organen nach einer Hypothese von Schayer als ein Mechanismus der protektiven Wirkung der Glucocorticoide im Endotoxinschock gelten kann, wurde in zwei weiteren randomisierten, kontrollierten Studien untersucht. Im Widerspruch zur Hypothese wurden im unbehandelten Endotoxinschock zum Zeitpunkt des Todes:

eine organspezifische (Veränderungen der HDC-Aktivität nur in 3 von 6 Organen).

eine uneinheitliche (Anstieg der HDC-Aktivität in Lunge und Leber, Abfall dagegen im Magen) und
-
3.
eine zeitabhängige (Anstieg in Lunge und Leber nur bei Tieren mit Tod zwischen 8–14 h, nicht bei Tod >20 h) Veränderung der HDC-Aktivität gemessen.
Im mit 5 und 50 mg MP/kg KG behandelten Endotoxinschock konnte, wegen der in dieser Studie nahezu gleichen Wirksamkeit bezüglich der ÜLR (90- 100%), der Einfluß auf die HDC-Aktivität erst zum Zeitpunkt 96 h nach Schockauslösung untersucht werden. Mit 5 mg MP/kg KG konnte in Lunge und Leber eine Hemmung, im Magen eine Aktivierung gegenüber einer Kontrollgruppe (NaCl) gemessen werden; die mit 50 mg MP/kg behandelten Tiere zeigten keinen Unterschied zur Kontrollgruppe. Zur Sicherung des Effektes sind weitere Untersuchungen zu einem früheren Zeitpunkt in der Schockentwicklung notwendig.
