Abstract
The reconstruction of large skeletal defects presents a challenging problem to the orthopedic community. The treatment of these defects has evolved significantly during the past two decades [14]. The recent, increased demand for bone grafts for reconstruction in limb salvage surgery for bone tumors and for reconstruction of failed arthroplasties has renewed interest in bone graft surgery. The types of grafts clinically available for use are nonvascularized auto- and allografts. Allografts (formerly homografts) are taken from a different individual of the same species, whereas autografts are harvested from the same individual. Xenografts (formerly heterografts) are grafts from a different species and are rarely used [58, 59]. Cellular elements in allografts are dead, due to processes that are used to diminish the immune response [50, 98]. Fairly convincing data support the concept that only a few cells on the surface of autografts survive, most undergoing necrosis [37, 82, 87].
