Zusammenfassung
Für alle Kinder stellt jeder diagnostische oder operative Eingriff ein unangenehmes, häufig traumatisierendes Ereignis dar, auf das es physisch und psychisch vorbereitet werden muß. Zu einer altersgerechten Vorbereitung gehört auch der adäquate Einsatz bestimmter Pharmaka, welche die kleinen Patienten vor dem „Streß“-Ereignis abschirmen (Prämedikation) und/oder den operativen Eingriff erleichtern sollen (Induktion einer Narkose). Benötigt werden dazu Substanzen, die angst- und spannungslösend (anxiolytisch), sedierend und hypnotisch-narkotisch wirken (Coté 1994). Diese therapeutischen Ziele sind häufig von den verwendeten Dosen und ihren Applikationswegen abhängig. Günstig sind Substanzen, deren Dosis-Plasmakonzentration-Wirkungs-Beziehungen nicht zu steil verlaufen und einen Sicherheitsabstand zu den unerwünschten Arzneimittelwirkungen (UAW) aufweisen (große therapeutische Breite). Dabei ist zu berücksichtigen, daß diese Beziehungen eine weite interindividuelle Variabilität aufweisen, d. h. daß es nicht eine Standarddosis und eine wirksame Zielkonzentration gibt, sondern nur „therapeutische“ Bereiche, denen man sich durch Dosistitration und Monitoring der Wirkung nähern sollte (Swerdlow u. Holley 1987).
