Zusammenfassung
Vor fast hundert Jahren formulierte Paul Ehrlich sein Postulat von der “selektiven Toxizität” antimikrobieller Wirkstoffe. Ein optimales Chemotherapeutikum soll demnach nur mit der Zelle des Krankheitserregers in Wechselwirkung treten und die Strukturen des Makroorganismus unbeeinflußt lassen. Somit unterscheiden sich antimikrobielle Arzneimittel von allen anderen Medikamenten, deren primäres Ziel es ist, spezifische Veränderungen durch Wechselwirkung mit dem Makroorganismus zu bewirken. Wenn es gelungen wäre, die Ehrlichsche Forderung zu erfüllen, brauchte man sich keine Gedanken über “Nebenwirkungen von Antibiotika” zu machen. Leider wird die theoretische Forderung von keiner in Frage kommenden Substanz erfüllt — tatsächlich beeinflussen alle bekannten Wirkstoffe auch den Wirtsorganismus. Damit entsteht die bekannte pharmakologische Dreierbeziehung zwischen Erreger, Patient und Chemotherapeutikum (Abb. 1).
