Abstract

This chapter deals with the general physiologic, pathophysiologic, and anatomic characteristics of cell transplantation. Like all other types of tissue transplantation, the outcome of cell engraftment is controlled by histocompatibility and MLC antigens (see Chapters 1 and 3); the major systems are summarized again in Figure 1. Each system is composed of antigen groups which vary from individual to individual. They are controlled by genetic loci on the chromosome (autosome), and since we are dealing with chromosome pairs, there are two alleles for each gene locus. The possible allelic (haplotype) combinations for a simple genetic locus in the offspring of two parents are shown in Figure 2. It may be seen from this figure to what extent tissue typing is necessary in a family to obtain not only the antigenic phenotype of an individual but also the genotypic penetration of a certain antigen combination within the family. According to the antigenic relatedness (HLA system) between donor and recipient we refer to autotransplantation of cells, isotransplantation, allotransplantation, and xenotransplantation. Autotransplantation refers to grafting within one individual, isotransplantation to grafting within two individuals of the same species who are practically isogenic (such as identical twins), allotransplantation to grafting between two nonidentical individuals from the same species, and xenotransplantation to grafting between two individuals from different species. In addition, blood group antigens must be considered when blood is transfused.
