Zusammenfassung
Die Kleinschnitt-Kataraktchirurgie mit flexiblen Linsen führt zu einer Verringerung des induzierten Hornhautastigmatismus. Pmma-Linsen mit 5-mm-Optik sollen ähnlich gute Astigmatismusresultate erzielen wie die durch einen 4 mm breiten Wundschnitt implantierten Weichlinsen. Die Anwendung der Ohne-Naht-Technik soll bei beiden Methoden zu einer weiteren Optimierung führen. Die Unterschiede im Astigmatismusverhalten wurde bei den oben genannten zwei Verfahren untersucht. Die Auswertung unserer Ergebnisse zeigt in beiden Gruppen eine geringe Veränderung des präoperativen Zylinderwertes. Der vertikale Frühastigmatismus wurde eliminiert. Ein schon in der Frühphase eintretender horizontaler Astingmatismus war in der 4-mm-Gruppe ein seltenes Phänomen. In der 5-mm-Gruppe hingegen, war diese Veränderung wesentlich öfter zu beobachten, wobei der Zylinderwert in diesen Fällen nicht größer als 0,75 dpt war. Zusammenfassend zeigt die bei der 5-mm-Gruppe öfter eintretende deutliche Achsendrehung, daß der Erhalt der Hornhautarchitektonik bei der 4-mm-Gruppe besser gewährleistet ist.
Summary
Small incision cataract surgery with flexible lenses results in a reduction of induced postoperative astigmatism. With “small optic” (5 mm) Pmma lenses one would expect similar astigmatism results. No-stitch techniques should optimize astigmatic changes with both lens types. We compared the difference of postoperative astigmatic change with both procedures. 100 consecutively operated eyes with a 4 mm incision cataract surgery and 40 consecutively operated eyes with a 5.2 mm incision cataract surgery were included in the study. Corneal astigmatism was measured with a Zeiss keratometer. Measurements were made preoperatively and 1-2 days, one week, one month and three months postoperatively. For both groups, mean astigmatism change was found to be minimal. A with the rule peak was not found. Early horizontal shifting was rare in the 4 mm group. In contrast, early astigmatism shifting was much more common in the 5 mm group. In the latter group the amount of early against the rule change of astigmatism did never exceed 0.75 dpt. In conclusion, the more frequent early against the rule shifting of the 5 mm group showed that the 4 mm group better maintains stability of the corneal architecture.
