Zusammenfassung
Der Chirurgischen Universitätsklinik Köln-Lindenthal wurden seit dem 1. 1. 1964 822 Patienten mit einem Karzinom der Speiseröhre zur ambulanten oder stationären Behandlung zugewiesen. Nur 34 von ihnen (4,1%) haben bisher nach Stellung der Diagnose 5 Jahre überlebt. Angesichts dieser Zahl ist unser primäres Therapieziel bei der Behandlung des Speiseröhrenkarzinoms die rasche Wiederherstellung der Schluckfunktion durch Verfahren, die dem Patienten ein Maximum an Lebensqualität ermöglichen. Wenn immer möglich, streben wir die Resektion und den Ersatz in einer Sitzung an. Kontraindikationen für die Resektion sind:

a)
ein erheblich reduzierter Allgemeinzustand des Patienten,
-
b)
Organmetastasen, deren Entfernung zur substantiellen Ausweitung des Eingriffs führen würde,

c)
die ausgedehnte Infiltration von Nachbarorganen,
-
d)
fixierte intraabdominelle Lymphknotenpakete.
