Zusammenfassung
Hämoblastosen und maligne Lymphome (ML) gehen infolge therapiebedingter Verlängerung der Überlebenszeit der Patienten mit zunehmender Beteiligung von ZNS, Rückenmark, Spinalwurzeln und peripheren Nerven einher (9, 17, 18, 19, 23, 32, 40). Peripher-nervöse Ausfälle können bedingt sein durch 1) neoplastische Infiltration oder Tumorkompression (Lym-phom, Chlorom), 2) nicht metastatische (paraneoplastische) Syndrome, 3) entzündliche Syndrome (Guillain Barre, Herpes Zoster), 4) Dyspro-teinämien, Amyloidablagerungen, 5) Therapieeffekte (Chemotherapie, Bestrahlung). Die für die Prognose und Therapie wichtige Abgrenzung der verschiedenen Ursachen für die Ausfälle des peripheren Nervensystems (PNS) ist klinisch oft schwierig oder unmöglich. So sind Guillain-Barré (LGL) artige Syndrome bei M. Hodgkin (22,31,37) und bei Non-Hodgkin-Lymphomen (NHL) (14, 15) klinisch kaum von einer Meningeose zu unterscheiden, ziehen aber unterschiedliche therapeutische Konsequenzen nach sich. Querschnittssyndrome können durch Tumorinfiltration bei M. Hodgkin (4), NHL und Leukämie (17, 30) oder durch nekrotisierende Mye-lopathien (16, 29) bedingt sein. Seltene nukleäre oder spinale Atrophien (28) entsprechen einer subakuten Motoneuropathie (33, 39) deren Beziehung zur amyotrophen Lateralsklerose (ALS) umstritten ist (5).
