Zusammenfassung
Die diabetische Nephropathie ist ein durch Bluthochdruck, Proteinurie und fortschreitende Niereninsuffizienz gekennzeichnetes Syndrom (Ritz u. Orth 1999). Die Entwicklung einer diabetischen Nephropathie ist eine gravierende, die Lebenserwartung des diabetischen Patienten begrenzende Komplikation, wobei der Patient sowohl durch das Risiko der Niereninsuffizienz als auch durch die begleitende hohe kardiovaskuläre Mortalität bedroht ist. Das renale Risiko ist bei Typ-1-und Typ-2-Diabetikern gleich hoch. Da in den letzten Jahren die Lebenserwartung von Diabetikern mit Nephropathie kontinuierlich zugenommen hat, erreicht ein immer größerer Prozentsatz älterer Typ-2-Diabetiker das Stadium der terminalen Niereninsuffizienz. Gegenwärtig liegt bei etwa 50% aller zur Nierenersatztherapie in Deutschland aufgenommenen Patienten ein Diabetes vor (Ritz et al. 1999; Schwenger et al. 2001).
