Zusammenfassung
Die Inzidenz von Wundheilungsstörungen und Weichteildefekten als Komplikation nach Knietotalendoprothesen wird mit bis zu 20 % aller Fälle angegeben. Es ist anzunehmen, dass dieser Prozentsatz im Rahmen von Revisionseingriffen am Kniegelenk angesichts des wiederholten und erweiterten Weichteilschadens noch weiter ansteigt. Mit zunehmender Anzahl implantierter Prothesen ist auch ein Anstieg komplexer Revisionsfälle unausweichlich. Defekte der Weichteile unterschiedlichen Ausmaßes bis hin zu freiliegendem Knochen oder Prothesenmaterial treten dabei in etwa 5 % der Fälle auf und stellen mitunter die schwerstwiegenden Komplikationen dar. Häufig ist aufgrund der folgenden Infektion ein Erhalt des prothetischen Materials nicht möglich und eine Arthrodese oder Amputation die einzig verbleibende Therapieoption. Entsprechend kommt in der Revisionschirurgie neben der Behandlung von Knochendefekten insbesondere dem Weichteilmanagement eine außerordentliche Bedeutung für den endgültigen Therapieerfolg zu.
