Rückenschmerzen sind keine Krankheitsentität, sondern ein Symptom. Der zugrundeliegende Pathomechanismus reicht von (seltenen)spezifischen Ursachen bis zu (normalen)Befindlichkeitsstörungen als Zeichen körperlicher Beanspruchung. In den meisten Fällen handelt es sich um rezidivierende Schmerzzustände, die sich nicht auf einen spezifischen Krankheitsprozess zurückführen lassen. Bei der Chronifizierung spielen psychosoziale Faktoren eine wesentlich größere Rolle als Röntgenbefunde und die medizinische Diagnose. Bereits bei akuten Rückenschmerzen bestimmen kognitive Prozesse im Sinne von Krankheitsbewertungen über das dadurch bedingte Verhalten den weiteren Krankheitsverlauf. Eine frühe Aktivierung kann bei akuten Rückenschmerzen chronische Verläufe verhindern.
