Der Wahn ist unter den Symptomen der Schizophrenie eines der prominentesten und faszinierendsten Phänomene. Seit Beginn der Erforschung psychischer Erkrankungen haben Wissenschaftler versucht, Theorien zur Entstehung und Aufrechterhaltung von Wahnsymptomen aufzustellen. Die phänomenologische Charakterisierung der Psychopathologie und Befunde aus Verhaltensstudien wurden seit der Entstehung der sog. „Dopamin-Hypothese“ der Schizophrenie zunehmend durch tierexperimentelle und pharmakologische Untersuchungen zu den neurobiologischen Grundlagen des Wahns ergänzt. In den beiden letzten Jahrzehnten gelang es, mit Hilfe bildgebender Verfahren auch die neuralen Korrelate einzelner Wahnsymptome ansatzweise zu identifizieren. Im Folgenden sollen die wesentlichen Konzepte und Theorien mit Bezug auf die bisher vorliegenden Befunde zur Bildgebung bei Wahnsymptomen vorgestellt werden.
