Abstract
Noninvasive ventilation (NIV) is used extensively today in the intensive care unit and in many non-intensive-care settings. NIV treatment has increased in recent years, with the goal of preventing intubation and invasive mechanical ventilation in a substantial number of patients. Nevertheless, a certain percentage of patients in whom NIV treatment fails require intubation. To date, extracorporeal membrane oxygenation (ECMO) in patients undergoing NIV treatment is mostly used as an alternative bridging strategy to IMV in patients awaiting lung transplantation. Furthermore, the use of awake ECMO to prevent intubation could also be a valid therapeutic choice in some patients in the early stages of acute respiratory distress syndrome. The awake ECMO strategy associated with NIV treatment should be managed in highly experienced centers.
