Abstract
Congenital vertical talus (CVT) is a complex deformity with varying etiology and spectrum of severity. The first line of management includes serial plasters by reverse Ponseti technique followed by minimal invasive surgery to reduce and pin talonavicular joint combined with a percutaneous/open Achilles tendon lengthening. Infrequently, in some cases, open procedures might have to be resorted to. CVT can be corrected by various approaches: anterior, posterior (Cincinnati), or combination of medial/lateral/posterior. The structures released are the same irrespective of the approach (described in detail in the chapter). Each approach has its own advantages and disadvantages. Considering the extensiveness of surgical dissection required, extensive release is a major undertaking on a small foot with associated complications.
This chapter not only describes the patho-anatomy and surgical technique in detail but also highlights tools in the assessment of results and possible complications of the surgery. The senior author has explained his approach through clinical cases.
