Abstract
Pancreas transplantation is a dynamic surgical field that is expanding, both in the complexity of surgical procedures as well as the indications for transplantation. The breadth of patients presenting for pancreas transplantation is far wider than for other candidates of solid-organ transplants due to the frequency where pancreas transplants are added to transplantation of an additional solid organ and the widespread pathophysiology associated with chronic pancreatic insufficiency. These two caveats demand highly specialized care from the anesthesiologist approaching pancreas transplantation. Not only must they be sensitive to subtle clinical changes suggesting end-organ disease secondary to pancreatic insufficiency in thoracic as well as abdominal organs, but they must also be very facile in the operating theatre to adjust their management to address a wide range of intra-operative events. Successful peri-operative care in the performance of pancreas transplantation requires a comprehensive understanding of the procedure to perform meticulous pre-operative preparation combined with clinical resilience intra-operatively. This chapter explores the core principles necessary for successful peri-operative care for the patient undergoing pancreas transplantation.
