Methods and Standards of Clinical Ethics Consultation

Abstract

Since the 1970s, health care has made giant steps toward formalization and professional policies regarding ethical reinforcement in health care. However, certain pivotal facets of health care ethics are still in need of modification. The issues surrounding clinical care are immense. However, an overarching resolution that connects the clear majority of these and other issues surrounding clinical care involves effective clinical ethics consultation methods. Health care ethics consultation(s) (HCEC) includes conversations with patients and families, attorneys, chaplains, social workers, and case management staff, all with the common purpose of value facilitation. This chapter examines existing methods of HCEC and their implementation in formal ethics consultation training.

HCEC seeks to facilitate discussion and develop resolution strategies between stakeholders. Typically, these stakeholders have difficulty resolving a value-laden issue about patient care. Through value facilitation, ethicists seek to bolster communication between health care professionals, patients, surrogates, and team members. The chapter begins with a standard formulation of clinical ethics, followed by a clinical case description presented thematically throughout the first half of the discussion. We then discuss the foremost contemporary clinical consultation approaches. Developing a consultation method that weds the most beneficial aspects of existing consultation methods requires a comparative analysis of existing clinical consultation methods. In doing so, the discussion attempts to develop a comprehensive consultation method from contemporary methods’ fragments. The second half of the chapter presents a detailed description of the ASBH’s efforts to standardize a credentialing process for clinical ethics consultants. This description identifies components of an ethics curriculum broad enough to accommodate various expertise levels while simultaneously identifying necessary facets of professional ethics.
