Abstract
The introduction of serum calcium measurement in metabolic and renal panels has increased recognition of normohormonal hyperparathyroidism (NHHPT), the entity in which elevated serum calcium is accompanied by a normal but non-suppressed parathyroid hormone (PTH). Because of its biochemical subtlety, there are important diagnostic and management nuances to consider in approaching this subset of patients, most notably the exclusion of other causes of hypercalcemia. This chapter provides a comprehensive review of NHHPT, including presentation and symptoms, serologic and imaging evaluation, intraoperative challenges, and postoperative considerations. The goal of parathyroid surgery in this cohort is normocalcemia.
