Abstract

Transanal total mesorectal excision (taTME) for rectal cancer is still in its infancy, although it has been introduced in its current setup and technique nearly a decade ago. Notwithstanding, several thousand patients have undergone the procedure worldwide since the first published case in 2010. The number of publications continues to grow, with the focus on the short-term outcomes. Understandably, concerns about the procedure’s safety and feasibility have been the main theme of initial publications. However, the potential to achieve the goal of this surgery, which is to cure patients of rectal cancer with less technical challenge, is still being investigated. At this moment, no sufficient data are available regarding long-term outcomes. Most studies have focused on the surrogate markers of oncologic quality, like grading of the resected specimen and percentage of involved resection margins. The definitive answer of the long-term outcomes like local recurrence rates and distant metastases is still awaited. This chapter provides a brief review of the available evidence to date for the pathologic and oncologic outcomes for patients undergoing taTME for curative-intent rectal cancer surgery.
