Percutaneous Management of Carotid and Vertebral Artery Disease

Abstract

CAS in suitable patients with the use of an EPD is a well-validated alternative to surgical revascularization for stroke prevention in patients with carotid artery stenosis. For optimal outcomes, all patients undergoing CAS need to be carefully selected, based on readily available clinical and angiographic features. Proximal embolic protection offers a strategy that will likely dramatically improve the safety of CAS. Patients who have symptomatic vertebral artery stenosis and have failed medical therapy should be considered candidates for percutaneous intervention in experienced hands. Larger trials of patients with vertebral artery stenosis are needed, both to further understand the natural history of the disease and to validate currently available treatment options.
