Fistules recto-vaginales des deux tiers inférieurs du vagin

Résumé

Les fistules recto-vaginales d’origine obstétricale sont exceptionnelles en France et dans les pays à haut niveau sanitaire. Elles sont très fréquentes en Afrique (plusieurs millions) et entraînent non seulement des répercussions médicales mais aussi sociales du fait de l’isolement de ces patientes souvent jeunes et rejetées par le milieu familial. Le diagnostic est confirmé par l’examen clinique. Le bilan sphinctérien nécessite en revanche une évaluation associant échographie endo-anale et manométrie. Les étiologies sont parfois congénitales le plus souvent acquises: traumatiques, infectieuses, oncologiques, mais surtout obstétricales. Leur traitement est chirurgical.
