Abstract

This chapter reviews systemic conditions that cause macroscopic and microscopic mucosal changes such as gastritis, which may be clinically and histologically distinct from primary inflammation. A gastric mucosal biopsy unifies the findings of the endoscopist and the histopathologist and may identify a systemic inflammatory disorder or vasculitis, infiltrative disease, or neoplasia. A number of systemic diseases interfere with smooth muscle or neuronal function and are associated with motility disorders of the stomach and delayed gastric emptying (gastroparesis). Gastric neoplasias are rare and almost always unexpected findings; however, it is important to be aware of conditions that predispose to gastric cancer such as primary immunodeficiencies, organ or bone marrow transplantation, and hereditary polyposis syndromes.
