Role of Exercise in the Management of Myositis

Abstract

Exercise in myositis has been controversial due to fear of increased disease activity. However, during the last several years, evidence has emerged supporting that exercise not only improves physical capacity and self-reported health but also can reduce disease activity and inflammatory markers in serum as well as in muscle tissue in adult polymyositis (PM) and dermatomyositis (DM). All small studies on exercise in inclusion body myositis (IBM) conclude that exercise is safe as to disease activity and inflammation; however, evidence on the response of exercise on muscle strength and physical capacity is somewhat diverging. Exercise is also safe in patients with juvenile dermatomyositis (JDM) with positive effects on functional capacity. Today, exercise including hand exercise, adapted to disease activity and patient’s fatigue and pain, is an important part of treatment in all patients with myositis. Exercise is one of the few treatment modalities that could improve physical capacity in individuals with IBM. Exercise adapted to the disease and performed logically is medicine without side effects.
