Zusammenfassung
Die Indikation zur arthroskopischen Rekonstruktion des hinteren Kreuzbandes (HKB) kann bei einer symptomatischen hinteren Instabilität mit einer posterioren Translation > 8 mm im Seitenvergleich gestellt werden. Begleitinstabilitäten der posterolateralen oder posteromedialen Gelenkecke sollten mitbehandelt werden. Die Operation beginnt mit der Entnahme der Semitendinosus- und Gracilissehne. Nach einer diagnostischen Arthroskopie und Therapie möglicher intraartikulärer Begleitschäden erfolgt das Bohren des femoralen Tunnels über ein tiefes anterolaterales Portal. Dann wird eine Verbindung zwischen anteriorem und posteromedialem Kompartiment präpariert und ein posteromediales Portal angelegt. Über dieses wird die tibiale HKB-Insertion unter Schonung der Restfasern des HKB präpariert. Über ein anteromediales Portal wird ein spezielles HKB-Zielgerät mit einem geschlossenen Zielhaken im tibialen Insertionsgebiet des HKB platziert und ein Führungsdraht gebohrt. Es erfolgt das Bohren des tibialen Tunnels und der Einzug des Transplantates. Die femorale Fixation erfolgt über einen Kippanker und die tibiale Fixation erfolgt als Hybridfixation mit einer resorbierbaren Interferenzschraube und einem Metallknopf.
Während der Nachbehandlung werden eine Teilbelastung sowie das Tragen einer speziellen HKB-Schiene für 6 Wochen empfohlen.
Abstract
Arthroscopic reconstruction of the posterior cruciate ligament (PCL) is indicated in patients with symptomatic posterior instability with a posterior translation > 8 mm. Accompanying instabilities of the posterolateral or posteromedial joint corner should also always be treated. The operation begins with harvesting of the semitendinosus and gracilis tendons. After diagnostic arthroscopy and arthroscopic treatment of possible concomitant articular damage, the femoral tunnel is drilled through a deep anterolateral portal. A connection between the anterior and posteromedial compartments is then prepared, a posteromedial portal is created, and debridement at the tibial PCL insertion is performed without damaging the remaining fibers of the PCL. A special PCL-aiming device with a closed target hook is placed in the tibial insertion area of the PCL via the anteromedial portal and a guide wire is drilled. After drilling the tibial tunnel, the PCL graft is inserted. Femoral fixation is via a toggle button and tibial fixation is a hybrid fixation with an absorbable interference screw and a metal button. Partial weightbearing and a special PCL brace with posterior support for 6 weeks is recommended during follow-up.
