Zusammenfassung
Patellafrakturen zeigen wie alle kniegelenknahen Frakturen eine niedrige Inzidenz. In den letzten Jahren zeigt sich auch bedingt durch den demografischen Wandel eine Zunahme an komplexen Frakturen mit einem Altersgipfel bei Patientinnen über 70 Jahre. Dies ist sicherlich auch auf die Überarbeitung der Leitlinie und der damit verbundenen großzügigeren Anfertigung von Computertomographien (CT) zur präoperativen Bestimmung der exakten Frakturmorphologie zurückzuführen. Die Versorgungsrealität in Deutschland sieht nach wie vor so aus, dass allen bekannten Nachteilen zum Trotz die klassische Zuggurtungsosteosynthese mit Kirschner-Drähten das Osteosyntheseverfahren der Wahl in der Behandlung von Patellafrakturen ist. Aufgrund der biomechanischen Vorteile der winkelstabilen Plattenosteosynthese gerade für komplexe, osteoporotische Frakturen zeichnet sich in den letzten Jahren ein Trend hin zu dieser Versorgung ab. Perspektivisch kann dies zu einer Abnahme der Revisions- und Komplikationsrate und somit mittelfristig zu einer volkswirtschaftlichen Entlastung führen.
Abstract
Fractures around the knee, such as patella fractures, are infrequent. However, due to demographic changes, there has been an increase in complex fractures of the patella, peaking in women over the age of 70. This could also be due to the updated treatment guidelines and the increased use of preoperative computed tomography (CT) scans to assess the exact morphology of the fracture. Disregarding the disadvantages of tension band wiring, it still remains the preferred option in Germany to treat complex patella fractures. The biomechanical advantages of locked plating especially for complex, osteoporotic fractures has led to a trend towards angular stable fixation. In the long term, this could lead to fewer complications and revisions, thus, decreasing the economic burden of healthcare.
