Zusammenfassung
Die Implantation einer Knietotalendoprothese (Knie-TEP) ist ein erfolgreiches standardisiertes Operationsverfahren. Trotzdem gelingt es nicht, an die hohe Patientenzufriedenheit von über 95 % nach Hüftendoprothesenimplantation anzuknüpfen. Im Jahr 2018 wurden laut Endoprothesenregister (EPRD) in Deutschland 119.131 Primärimplantationen und 13.378 Folgeeingriffe nach Knie-TEP durchgeführt. Das entspricht 11 % Revisionsoperationen. Damit liegt der Prozentsatz an Revisionsoperationen nach primärer Knie-TEP höher als in Schweden (<6 %) oder in den USA (<7,5 %). Aktuell zählen laut Register- und Studiendaten zu den häufigsten Revisionsursachen nach primärer Knie-TEP die aseptische Lockerung, die Infektion sowie die Instabilität. Dabei ist in Deutschland die Anzahl der Folgeoperationen wegen einer Infektion von 22,3 % im Jahr 2017 auf 14,7 % im Jahr 2018 gesunken. Im Vergleich dazu stiegen in Schweden und den USA die Revisionsoperationen in Folge einer Infektion an. Der Beitrag gibt einen Überblick über die häufigsten Versagensursachen nach primärer Knie-TEP in den letzten 20 Jahren. Dabei werden die Daten aus Deutschland mit anderen Registerdaten sowie der aktuellen Literatur verglichen. Zusätzlich wird ein Vergleich der aktuellen Daten mit älteren Studien und Registerdaten vor 2000 vorgenommen.
Abstract
Total knee arthroplasty (TKA) has become a successful and highly standardized procedure. Nevertheless, the high satisfaction level of 95–98% after total hip arthroplasty has not yet been achieved. According to the German Arthroplasty Registry (EPRD) 2019, a total of 119,131 primary TKA and 13,378 revision arthroplasties were performed in Germany in 2018. This corresponds to need for revision surgeries in around 11% of cases. That means the percentage of revision surgery after primary TKA in Germany is much higher than in Sweden (<6%) or the USA (<7.5%). According to registry and study data, the most common causes for revision after TKA include aseptic loosening, infection, and instability. It was found that the number of revision surgeries caused by infection decreased from 22.3% (EPRD 2017) to 14.7% (EPRD 2019) in Germany. In contrast increased revision procedures for periprosthetic infection were seen in Sweden and the US. The aim of this article is to provide an overview of the most common causes of failure after TKA in the past 20 years. Data from Germany are compared with registry data of other countries and current literature. In addition, current data are compared with older studies and registry data prior to 2000.
