The treatment strategies described in Fig. 1 for the patient groups “vancomycin susceptible; ABPC susceptible” and “vancomycin susceptible; ABPC resistant” were labelled incorrectly. This error only appeared in Fig. 1, all analyses and conclusions reported in the paper were not affected by this error. The revised Fig. 1 is given below. The original article has been corrected.
