Zusammenfassung
Tiefe sternale Wundinfektionen gelten als schwere Komplikation nach medianer Sternotomie. Ziel dieser Übersichtsarbeit war, den Einfluss der Vakuumtherapie auf ausgewählte Endpunkte bei Personen mit tiefen sternalen Wundinfektionen zu beschreiben. Die Literaturrecherche erfolgte mit den Suchbegriffen (Unterdrucktherapie, V.A.C®-Therapie, tiefe sternale Wundinfektion, mediane Sternotomie) im Zeitraum von Juni bis September 2019 sowie 2021 in den Datenbanken MEDLINE via PubMed, CINAHL® complete und Academic Search Elite via EBSCOhost. Die Auswahl und Bewertung der ausgewählten Studien (n = 11) wurden durch 2 unabhängige Personen (V.G., A. F.) mit den Bewertungshilfen von Behrens und Langer (2016, S. 198 ff) durchgeführt. Die Behandlungsdauer, Mortalität sowie die Reinfektion konnten unter Anwendung der Vakuumtherapie reduziert werden. Zudem besteht ein Trend zu einem kürzeren intensiv- sowie normalstationären Aufenthalt. Die Anwendung der Vakuumtherapie kann als Methode erster Wahl zur Behandlung der tiefen sternalen Wundinfektion mit Einfluss auf essenzielle Dimensionen bezeichnet werden.
Abstract
Deep sternal wound infections are considered a serious complication after median sternotomy. The aim of this review was to show the impact of vacuum assisted closure on selected outcomes in individuals with deep sternal wound infections. The literature search was conducted using the search terms negative pressure therapy, V.A.C® therapy, deep sternal wound infection, median sternotomy from June to September 2019 and 2021 in the databases MEDLINE via PubMed, CINAHL® complete, and Academic Search Elite via EBSCOhost. The selection and the evaluation of the selected studies (n = 11) were performed by two independent persons (V.G., A. F.) using the evaluation tools of Behrens and Langer (2016, p 198 ff). The duration of treatment, mortality and reinfection could be reduced using vacuum assisted closure. In addition, there is a trend towards a shorter intensive and normal inpatient stay. The use of vacuum assisted closure can be considered as the method of choice for the treatment of deep sternal wound infections affecting essential dimensions.
