Samenvatting
Deze oratietekst stelt aan de orde of psychotherapiepatiënten opknappen door specifieke behandeltechnieken of door zogenaamde common factors (zoals de kwaliteit van de werkrelatie). Specifieke behandeltechnieken ontlenen hun effect aan het feit dat ze aangrijpen op verstoorde cognitieve processen die aan psychische stoornissen ten grondslag liggen. De empirische evidentie en theoretische onderbouwing van specifieke behandeltechnieken zijn sterk. Ook de kwaliteit van de werkrelatie hangt consistent samen met het psychotherapieresultaat. Dat deze werkrelatie nodig is om individuele ontplooiing en zelfinzicht van patiënten te bevorderen, is niet zeker. Onderzoek naar sociale beïnvloeding en weerstand tegen verandering maakt aannemelijk dat een goede werkrelatie tot functie heeft patiënten in hun eigen veranderingsproces geïnvolveerd te laten raken. Patiënten moeten immers het zware werk doen. Juist op dit punt - patiënten bereid vinden om te veranderen - is het afwachten of kortere behandelingen in de generalistische ggz en eHealth-behandelingen niet tekort gaan schieten.
Notes
Onder psychotherapie versta ik alle gangbare vormen van psychologische behandeling.
Het idee dat men via zelfreflectie betere zelfkennis kan bereiken, is theoretisch weinig gefundeerd. Er is weinig bewijs voor het bestaan van onbewuste conflicten, noch voor het succesvol weghouden van informatie uit de bewuste aandacht gedurende een lange periode. Er zijn tal van onbewuste cognitieve processen, maar daartoe kunnen we (ook met training) nauwelijks bewuste toegang verkrijgen. De metafoor dat we ‘als archeologen in onszelf moeten graven’ is weinig adequaat. Adequater en ook zinvol voor nadere bestudering, is dat mensen een zelfconcept construeren en via geconstrueerde narratieven vertellen wie ze zijn. Common factors en werkrelatie worden in deze tekst vaak in één adem genoemd. Er is weinig consensus over wat de overige common factors precies zijn en hoe ze van de kwaliteit van de werkrelatie gescheiden moeten worden. Het gaat om zaken als veranderingsbereidheid, therapieverwachting en hoop. In deze rede is niet geprobeerd deze begrippen te ontwarren. Het is wel zinvol hier op te merken dat niet alle common factors gebaseerd zijn op sociale beïnvloeding of weerstand tegen sociale beïnvloeding. Er zijn ook algemene therapiefactoren die verandering bevorderen en weerstand verminderen, maar die meer te maken hebben met intrapersoonlijke leer- en implementatieprincipes (Keijsers e.a., 2012, 2013 ). Hier volgen enkele voorbeelden: Fysieke ‘cues’ (zoals een Post-it op de bureaulamp om op tijd pauze te nemen) zijn behulpzaam om op het juiste moment aan het juiste gedrag te denken. Tactische veranderingen in een situatie (zoals geen chips meer in huis hebben en etensresten weggooien) helpen om ongewenst automatisch gedrag te voorkomen. Realistische doelen stellen, die doelen concreet maken, implementatie-intenties gebruiken, grote stappen in kleine stappen opdelen en een tijdsplanning maken, helpen bij het acceptabel en bereikbaar maken van gedragsveranderingen. En het opwekken van emoties (mits niet te sterk) tijdens gesprekken bevordert associatief leren (Ledoux, 2002 ).
