Samenvatting
Achtergrond
Wanneer ouderen vanuit een residentiële naar een acute zorgsetting worden doorverwezen kan belangrijke informatie met betrekking tot de zorg zoek raken. Daarnaast neemt het concept vroegtijdige zorgplanning een steeds prominentere plaats in binnen de ouderenzorg, maar het blijft een uitdaging om de resultaten van dit proces over de verschillende zorgsettings heen te communiceren. De ontwikkeling van een goede methode voor informatietransfer is dus een sleutelelement bij de zorgoverdracht van de oudere patiënt.
Doel
Deze studie had als doel om een gestandaardiseerd transferdocument te ontwikkelen en te valideren binnen een samenwerkingsverband tussen de residentiële en acute zorgsector in Leuven, Vlaams-Brabant.
Methode
Na literatuurstudie werd een topiclijst gegenereerd die als basis diende voor een Delphi-ronde waaraan 16 experts uit zowel de residentiële als de acute zorgsector deelnamen. Daarna werd het document beoordeeld op inhoudsvaliditeit door een groep van negen experts uit de acute en residentiële zorgsetting. Tot slot werd de indruksvaliditeit beoordeeld door twee verpleegkundigen en twee artsen, at random gekozen uit bovenstaande settings.
Resultaten
Alle 44 subthema’s vertoonden een uitstekende inhoudsvaliditeit. De scale content validity universal agreement (S-CVIUA) voor het gehele transferdocument was 0,68. De average scale content validity (S-CVIAve) was 0,96. Na een tweede en laatste Delphi-ronde werd een definitief transferdocument opgesteld bestaande uit acht thema’s en 50 subthema’s.
Conclusie
Op basis van deze resultaten werd een gestandaardiseerd transferdocument ontwikkeld en gevalideerd.

Abstract
Background
When elderly patients are transferred from a residential to an acute care setting, important information regarding their health care can be lost. Over the past years, the concept of advance care planning has also been given a more prominent place in the care for the elderly. However it remains a challenge to communicate the results achieved by this process when patients are referred to another health care setting. Developing a sound method for transferring information is a key element in the transitional care for the elderly patient.
Objectives
In collaboration with the residential and acute care settings in Leuven, Flemish Brabant, Belgium this study aimed to develop a validated, standardized transfer-sheet.
Methods
After a literature search a topic list was generated to be used as the basis for a Delphi-procedure in which 16 experts from both the acute and the residential care settings participated. The transfer-sheet was then evaluated for content validity by an expert-panel (n = 9) from the acute and residential care settings. Face validity was assessed by two nurses and two doctors, randomly selected from the above settings.
Results
All 44 subthemes in the transfer-sheet showed excellent content validity. The scale content validity universal agreement (S CVIUA) for the entire transfer-sheet was 0.68. The average scale content validity (S CVIAve) was 0.96. After a second and final Delphi-round a final transfer-sheet was constructed consisting of 8 themes and 50 sub-themes.
Conclusions
Based on these results standardized transfer-sheet was developed and validated.

Behandeldoelen en therapiebeperkingscodes
Binnen het concept vroegtijdige zorgplanning worden meestal 3 categorieën van behandeldoelen gehanteerd [14]:

A = alles doen (levensverlenging): er wordt geopteerd voor maximale therapeutische zorg, hospitalisatie gebeurt zo nodig. Medische en verpleegkundige handelingen, kinesitherapie en ergotherapie zijn gericht op maximale revalidatie.

B = behoud van functies: er wordt gekozen voor therapie en hospitalisatie met als behandeldoel functiebehoud. Medische en verpleegkundige handelingen, kinesitherapie en ergotherapie zijn gericht op zo lang mogelijk behouden wat men nog kan, zonder intensief te revalideren.

C = comfortzorg: maximale zorg en therapie worden gegeven in functie van het comfort van de patiënt, de voordelen van de behandeling moeten opwegen tegen de nadelen. Medische en verpleegkundige handelingen, kinesitherapie en ergotherapie zijn gericht op het comfort van de patiënt.

C T = comfortzorg in een terminale fase: de reeds geboden comfortzorg zal in een terminale levensfase intensiever worden om een maximale symptoomcontrole te kunnen bewerkstelligen [24]
In de acute zorgsector wordt vaak gewerkt met therapiebeperkingscodes:
DNR 0: geen therapiebeperking
DNR 1: Niet reanimeren:
0 in geval van circulatiestilstand geen thoraxmassage, defibrillatie noch medicatie
0 in geval van respiratoir falen geen intubatie of kunstmatige beademing
