Zusammenfassung
Während es sich bei dem Phase-3-Schenkelblock um ein physiologisches Phänomen handelt, liegt beim beschleunigungsabhängigen Schenkelblock eine Schädigung des His-Purkinje-Systems vor. Eine klare Unterscheidung dieser beiden Formen ventrikulärer Aberranz ist deshalb von klinischer Bedeutung. Folgende Kriterien dienen der Differenzialdiagnose: kritische Herzfrequenz, Manifestationsart und Differenz zwischen der Zykluslänge beim Auftreten und Verschwinden des Blocks. Das funktionelle Unterscheidungsmerkmal zwischen den beiden ist die Abnahme der Refraktärzeit mit Frequenzanstieg bei der ersten Form und deren Zunahme bei der zweiten Form.
Abstract
While the phase 3 bundle branch block is a physiological phenomenon, the acceleration-dependent bundle branch block is caused by damage to the His-Purkinje system. Thus for clinical reasons an unambiguous differentiation of these two forms of ventricular aberrancy is mandatory. This differentiation is possible by using the following criteria: critical heart rate, mode of onset and difference between the cycle length at onset and resumption. The most important functional difference between the two forms of aberration is the shortening of the refractory period by heart rate increase with the phase 3 bundle branch block and its prolongation with the acceleration dependent brunch block.
