Les tumeurs stromales gastro-intestinales (GIST) sont les tumeurs mésenchymateuses digestives les plus fréquentes. Les GIST sont caractérisées par une hyperexpression d’une tyrosine-kinase (c-KIT). Nous rapportons une étude rétrospective portant sur 30 cas de GIST confirmées histologiquement et à l’immunohistochimie, colligés au CHU Mohammed-VI de Marrakech. L’âge moyen des patients était de 56 ans (de 23 à 80 ans). La symptomatologie révélatrice était une douleur abdominale dans la majorité des cas, suivie de la découverte d’une masse abdominopelvienne et de l’hémorragie digestive. La localisation gastrique était prédominante (11 cas), suivie par la localisation grêlique (sept cas), rectale (cinq cas), mésentérique (cinq cas) et duodénale (deux cas). 73,3 % des patients ont été traités par exérèse chirurgicale complète. À l’histologie, l’expression du marqueur c-KIT était présente dans 100 % des cas. Au bilan d’extension, la tumeur était localisée dans 70 % des cas, localement avancée dans 16,6 % et métastatique dans 13,3 %. Un traitement médicamenteux à base d’imatinib était entrepris en adjuvant dans 26,6 % des cas, et dans 16,6 % des cas en néoadjuvant. Avec un recul moyen de cinq ans, la survie moyenne était de 90 % à cinq ans, avec une rémission complète dans 46,6 % des cas, une rémission partielle dans 13,3 %, une récidive tumorale dans 16,6 % et le décès dans 10 % des cas.
Abstract
Gastrointestinal stromal tumors (GISTs) are the most common digestive mesenchymal tumors. GISTs are characterized by over-expression of the tyrosine kinase receptor KIT. We report a retrospective study of 30 cases of gastrointestinal stromal tumors confirmed histologically and immunohistochemically, collected at the University Hospital Mohammed VI of Marrakech. The average age of patients was 56 years (23–80 years). The revealing symptoms were abdominal pain in most cases, followed by the discovery of an abdominal–pelvic mass and gastrointestinal bleeding. Gastric localization was predominant (11 cases), followed by small bowel (7 cases), rectal (5 cases), mesenteric (5 cases), and duodenal (2 cases) locations. 73.3% of patients were treated with complete surgical resection. A histological marker expression c-KIT was present in 100% of the cases. The staging of the tumor shows a localized tumor in 70% of the cases, locally advanced in 16.6%, and metastatic in 13.3%. Medication with Imatinib was initiated in 26.6% of the cases in adjuvant and neoadjuvant in 16.6% of the cases. On a mean of 5 years, the 5-year survival was 90% and there were 46.6% cases of complete remission, 13.3% of partial remission, 16.6% of tumor recurrence, and 10% deaths.
