Conclusion

Mediastinal dermoids some times present in uncommon ways. Asymptomatic dermoids may increase in size enormously and present very late rupturing through the chest wall or into the pleural cavity. Dermoid may also grow enormously pressing the heart and causing cardiac failure. Traversing upwards and rupturing through the supra sternal area to present finally as a fleshy mass hanging in front of the sternum is also an unique presentation as observed in our case.
