Advanced therapy for prostatitis: Minimally invasive and invasive therapies

  • Published:
Current Urology Reports Aims and scope Submit manuscript

Abstract

The clinical syndromes of chronic prostatitis range from well-defined chronic bacterial infections to poorly defined chronic pelvic pain syndrome, previously referred to as prostatodynia and abacterial prostatitis. Faced by the obscure nature of the disease, its protracted course, and the poor response to oral medication, urologists have considered alternative treatment options. This article reviews the indications and outcomes of minimally invasive and invasive therapies for chronic prostatitis syndromes and discusses their potential use and benefits.

This controlled study demonstrated increased intraprostatic pressures in patients with Category III prostatitis compared with normal control subjects. Whether this elevated pressure is a cause or an effect of the pathologic process remains a matter of debate. However, this may provide insight into the potential beneficial effect of prostatic massage by decreasing intraprostatic pressure.

Thermotherapy proved to be more efficacious than the sham treatment. Microwave thermotherapy may be an effective concept for the treatment of patients with Category III prostatitis if larger well-designed studies confirm these results.

The conclusion was that TUNA in CPPS is comparable with sham treatment; therefore, it cannot be recommended as routine treatment of CPPS. It is interesting to appreciate the magnitude of the placebo effect (cystourethroscopy) in this study and to interpret noncontrolled studies very cautiously.
