Autoimmunity in chronic urticaria and urticarial vasculitis

Abstract

In contrast to acute urticaria, etiology cannot be identified in most cases of chronic urticaria. Recent evidence suggests that a subset of patients with chronic urticaria may have an autoimmune basis for their condition. The demonstration of antithyroid autoantibodies in some patients with chronic idiopathic urticaria (CIU) provides support for an association. However, the discovery of a positive skin test response to intradermal injection of autologous serum in as many as 60% of patients with CIU led to the identification of autoantibodies to IgE and the a-chain of the high-affinity IgE receptor, FceRIa. Additional studies have demonstrated that some of these autoantibodies are capable of releasing histamine from donor basophils and mast cells. This article reviews the literature that addresses a possible autoimmune etiology in a subset of patients with CIU. Urticarial vasculitis is differentiated from chronic urticaria based on clinical features and biopsy findings of leukocytoclastic vasculitis. Most cases of urticarial vasculitis are secondary to an underlying systemic disease. The presence of autoantibodies has also been demonstrated in a subset of patients with primary urticarial vasculitis. This article briefly reviews some of this data.

Patients with CIU and other autoimmune diseases were evaluated for the presence of anti-FceRIa using an ELISA-based assay. Although these autoantibodies can be found in other autoimmune diseases, only sera from patients with CIU and anti-FceRIa autoantibodies released histamine. The IgG subtype of the anti-FceRIa autoantibodies in CIU was further characterized as IgG1 and IgG3, and the authors present data suggesting a role for complement.

The role for complement was investigated using histamine release from dispersed mast cells in response to untreated sera, heated sera (decomplemented), or isolated IgG from sera of patients with CIU. Only sera of patients with CIU or isolated IgG of patients with CIU combined with normal serum were able to release significantly more histamine than the control.

Using sera from 15 patients with CIU and a positive ASST, it was demonstrated that 1) 14 sera had anti-FceRIa autoantibodies, 2) only sera from two patients released histamine from donor basophils, and 3) the ASST in these patients was unaffected by heat-treated/IgG-depleted sera. This study suggests that a positive ASST may be due to other HRFs present in the sera of CIU patients.

In this initially double-blind study, cyclosporin was used to treat patients with CIU. Eight of 19 patients initially responded to 4 weeks of therapy, but six of the eight relapsed when therapy was discontinued. Only one was still clear at 6 months. All nonresponders and placebo patients were offered open-label cyclosporin. Eleven of 17 responded, with four remaining clear/almost clear at 6 months. Treatment options are offered in this article.
