Zusammenfassung
Vorhofflimmern ist die häufigste anhaltende Herzrhythmusstörung des Erwachsenen. Bei Vorhofflimmern ohne organisches Herzleiden gelten die 1 c-Antiarrhythmika Flecainid und Propafenon als Mittel der Wahl. Bei vorbestehender koronarer Herzkrankheit wären Betablocker als erstes anzuwenden, bei ausgeprägter linksventrikulärer Dysfunktion ist Amiodaron angezeigt. Bei einer Tachyarrhythmia absoluta ist zunächst eine Normalisierung der Kammerfrequenz durch den Einsatz von Kalziumantagonisten, Betarezeptorenblockern, Digitalsglykosiden oder Amiodaron sowie anderen Antiarrhythmika zur Senkung der AV-nodalen Leitungskapazität anzustreben. Sprechen die Patienten auf die medikamentöse Therapie nicht an, so steht als wirksame Maßnahme die Ablation des AV-Knotens mit konsekutiver Schrittmacherimplantation zur Verfügung. – Bei der häufigen Form des anfallsweisen – also paroxysmalen – Vorhofflimmerns hat das neue Konzept der Pill-in-the-pocket-Strategie besonderes Interesse gefunden. Diese besagt, dass die in der Tasche mitgeführte antiarrhythmische Medikation anfallsbezogen einzunehmen ist. Das Prinzip besteht darin, bei paroxysmalem Vorhofflimmern keine dauerhafte Medikation zu verabreichen, sondern Rhythmusmittel ereignisbezogen durch den Patienten selbst einnehmen zu lassen mit dem Ziel der unmittelbaren Regularisierung der Herzschlagfolge. Hier werden vor allem Flecainid und Propafenon empfohlen. Von speziellem Interesse ist auch die Kombination von pharmakologischen und elektrotherapeutischen Behandlungsstrategien, wie sie bei der sog. Hybridtherapie Verwendung finden. Die modernen elektrischen sowie medikamentösen, in ständiger Weiterentwicklung befindlichen Therapieoptionen dienen dem Ziel, den Patienten durch individuell angepasste therapeutische Maßnahmen die Rückkehr in das gewohnte soziale Umfeld zu ermöglichen, verbunden mit der gebotenen Alterskompetenz bei hoher Lebensqualität.
Abstract
Atrial fibrillation (AF) is the most common arrhythmia in adults. The pharmacological treatment of AF still remains challenging in daily practice. The recently published guidelines of the American College of Cardiology (ACC), the American Heart Association (AHA) and the European Society of Cardioloy (ESC) clearly outline therapeutic approaches for different types of AF. Medical treatment is based on beta blockers and conventional antiarrhythmic drugs like Flecainide and Propafenon and non-antiarrhythmic drug therapy to prevent AF: “upstream” therapy including RAAS blocking agents (ACE inhibitors, angiotensin-receptor blockers), aldosterone antagonists, statins and polyunsaturated fatty acids (PUFA). Class 1 c antiarrhythmic agents (Flecainide, Propafenon) should be restricted to patients with no or minimal left ventricular dysfunction including hypertensive heart disease or mild heart failure. Amiodarone should be chosen in patients refractory to class 1 c drugs or those with already reduced left ventricular function. The “pill in the pocket” strategy can be used successfully in patients without structural heart disease and rare episodes of atrial fibrillation. Future studies in a large cohort of patients are still necessary to clarify the value of pharmacological and interventional therapies in atrial fibrillation.
