Hintergrund
Ursächlich für Bewegungseinschränkungen des Ellenbogengelenks können extrinsische oder intrinsische Ursachen sein.
Fragestellung
Die Indikation zur Arthrolyse des Ellenbogengelenks wird nach Ausschöpfung der konservativen Therapie ohne Befundbesserung gestellt, in der Regel nach 6–12 Monaten. Bei schweren Bewegungseinschränkungen mit einer globalen Bewegungsamplitude von < 60° sowie extrinsischen Ursachen, z. B. ausgeprägte heterotope Ossifikationen, ist die offene Arthrolyse der arthroskopischen zu bevorzugen.
Methoden
Bei der offenen Arthrolyse ist der Zugang am häufigsten lateral, kann bzw. muss jedoch in Abhängigkeit der Ursache oder bestehender Zugänge medial, bilateral oder dorsal erfolgen.
Ergebnisse
Die offene Arthrolyse des Ellenbogengelenks über einen lateralen Zugang erzielt bei extrinsischen Ursachen gute Ergebnisse bei niedriger Komplikationsrate. Bei intrinsischen Ursachen werden signifikant schlechtere Ergebnisse verzeichnet. Posttraumatische Ellenbogensteifen zeigen deutlich bessere postoperative Ergebnisse hinsichtlich des Gewinns an Bewegungsumfang, wenn mit degenerativen Ellenbogensteifen verglichen.
Schlussfolgerung
Die offene Arthrolyse des Ellenbogengelenks ist eine nach wie vor häufig eingesetzte Prozedur zur Behandlung der Ellenbogensteife und eignet sich besonders für posttraumatische, extrinsische Pathologien des Ellenbogengelenks.
Abstract
Background
Restricted mobility of the elbow can be caused by extrinsic or intrinsic causes.
Objectives
The indication for elbow arthrolysis is made after conservative therapy has been exhausted without any improvement in the findings, usually after 6–12 months. In the case of severe movement restrictions with a global movement amplitude of < 60° and extrinsic causes, e.g., heterotopic ossifications, open arthrolysis is preferable to arthroscopic.
Methods
In open arthrolysis, access is most commonly lateral, but can or must be medial, bilateral, or posterior, depending on the cause or existing access.
Results
Open elbow arthrolysis via a lateral approach achieves good results with low complication rates in the case of extrinsic causes. Significantly worse results are recorded for intrinsic causes. Posttraumatic elbow stiffness shows significantly better postoperative outcomes in terms of range of motion gain when compared to degenerative elbow stiffness.
Conclusion
Open elbow arthrolysis is still a frequently used procedure for the treatment of elbow stiffness and is particularly suitable for posttraumatic, extrinsic pathologies.
