Résumé
La rééducation joue un rôle majeur dans la prise en charge et le traitement médical ou chirurgical du canal lombaire rétréci. Le programme qui doit être adapté à chaque individu est fondé sur des principes simples dont l’objectif est de diminuer le conflit contenant-contenu au niveau du canal lombal. Les techniques de rééducation et de physiothérapie interviennent sur différents symptômes: contre la douleur, contre les troubles de la statique et de l’équilibre, contre les déficits de force, ou contre le déconditionnement à l’effort. Quand le traitement est médical de première intention, on recherchera une sédation de la douleur et le contrôle de la position lombaire optimale afin de limiter les douleurs et accroître le périmètre de marche. Dans un cadre postopératoire il faudra dépister les complications du traitement, respecter les délais de cicatrisation, puis optimiser la mobilité et les performances fonctionnelles des patients. Différentes modalités de la prise en charge sont précisées en fonction du type de chirurgie et des délais postopératoires.
Abstract
Rehabilitation plays a major role in the management, medical treatment and surgery of lumbar spinal stenosis. Treatment courses must be adapted to each individual according to basic principles aimed at mitigating the container-contents conflict in the lumbar canal. Rehabilitation and physiotherapy techniques impact various symptoms: pain, statics and balance disorders, strength deficits and deconditioning syndrome. When the primary focus of treatment is medical, the goal is pain relief and determining the optimal lumbar position that limits pain and increases walking distance. Postoperatively, it is necessary to detect treatment-related complications, respect the time needed for healing, and optimize the mobility and functional capabilities of patients. Various rehabilitation modalities are detailed based on the type of surgery and postoperative timeframes.
