Zusammenfassung
Partizipative Entscheidungsfindung („shared decision-making“) ist ein zentraler Aspekt der Patient:innenzentrierung und ein Schlüsselelement qualitativ hochwertiger Gesundheitsversorgung. Sie beschreibt einen Kommunikationsprozess zwischen Behandelnden und Patient:innen, bei dem auf Basis geteilter Informationen zu evidenzbasierten diagnostischen und/oder therapeutischen Verfahren eine gemeinsam verantwortete Entscheidung getroffen wird. Hierbei kommt den Präferenzen, Werten und Bedürfnissen der Patient:innen eine zentrale Rolle zu. Ziel dieses Artikels ist es, die Konzepte der Patient:innenzentrierung und partizipativen Entscheidungsfindung sowie praktische Hinweise zur Umsetzung in der onkologischen Routineversorgung zu erläutern. Des Weiteren werden aktuelle Studien zur Förderung der Umsetzung von partizipativer Entscheidungsfindung sowie Einflussfaktoren auf die Implementierung in der onkologischen Routineversorgung in Deutschland dargestellt. Obwohl die partizipative Entscheidungsfindung von vielen Patient:innen gewünscht und von der Gesundheitspolitik gefordert wird, ist sie weiterhin nicht flächendeckend in die onkologische Routineversorgung integriert. Barrieren für eine erfolgreiche Umsetzung partizipativer Entscheidungsfindung finden sich sowohl auf individueller Ebene als auch auf der Ebene von Gesundheitsorganisationen und des Gesundheitssystems. In den letzten Jahren wurde eine Reihe wirkungsvoller Interventionen entwickelt, die verschiedene dieser Ebenen adressieren. Dennoch bleibt die Umsetzung herausfordernd und komplex.
Abstract
Shared decision-making as part of patient-centeredness is a key element of high-quality healthcare. It describes a communication process between healthcare professionals and patients in which a shared decision is made based on shared information about evidence-based diagnostic and/or therapeutic procedures and the preferences, values, and needs of patients. The aim of this article is to explain the concepts of patient-centeredness and shared decision-making as well as to provide practical advice for implementation in routine cancer care. Furthermore, current studies on promotion of the implementation of shared decision-making as well as factors influencing implementation in routine cancer care in Germany are presented. Although shared decision-making is desired by many patients and demanded by health policy, it is still not widely integrated into routine cancer care. Barriers to successful implementation of shared decision-making can be found at the individual level as well as at the level of healthcare organizations and healthcare systems. In recent years, a number of effective interventions have been developed that address several of these levels. However, implementation remains challenging and complex.
