This note was written while the second author was visiting the first author in ConCoCO (Concordia Computational Combinatorial Optimization Laboratory). The research was undertaken, in part, thanks to funding from the Canada Research Chairs Program and by the China Scholarship Council. In addition to expressing their gratitude for this support, the authors want to thank the two anonymous referees for their thoughtful suggestions, which helped to improve the introductory section.
