Radiofrequency thermoablation – the neurosurgical perspective

Zusammenfassung

Bereits in der älteren Fachliteratur gibt es Berichte über die erfolgreiche Anwendung der stereotaktischen Radiofrequenz (RF)-Thermoablation für die Behandlung therapierefraktärer Epilepsie. RF-Sonden mit Zeit- und Temperaturkontrolle generieren ein scharf begrenztes Koagulat, dessen Größe mit gewissen Einschränkungen kontrollierbar ist. In Kombination mit hochauflösender MRT-Bildgebung ermöglicht diese Technik minimalinvasiv die Ablation nodulärer Heterotopien, kleiner fokaler kortikaler Dysplasien Typ II (FCDII) oder hypothalamischer Hamartome alternativ zur mikrochirurgischen Resektion.

Abstract

Even 40 years ago the medical literature reported the efficacy of stereotactic radiofrequency (RF) thermoablation for the treatment of therapy-refractive epilepsy. Using RF probes with time and temperature control, sharply demarcated clots could be generated, the size of which could be controlled within certain limitations. This technique in combination with high resolution magnetic resonance imaging (MRI) enables the minimally invasive, controlled ablation of nodular heterotopias, small focal cortical type II dysplasias and hypothalamic hamartomas as an alternative to microsurgical resection.
