Verbesserte Lebensqualität unter Levetiracetam-Monotherapie

Abstract

The leading aim of anticonvulsive therapy is to stop seizures. Nevertheless, the side effects of the anticonvulsant therapy have to be taken into account to assure that the patient's quality of life is not impaired. In this article the case of a 34-year old mother of two children is presented, who suffered from focal seizures due to an oligoastrocytom WHO II.

Combination therapy with carbamazepin and levetiracetam effectively reduced seizure frequency and severity. However, the patient was very tired, lost her strength and was too weak to cope with her daily agenda. After stopping therapy with carbamazepin and increasing the dose of levetiracetam, these symptoms ceased and the patient is now able to easily manage her daily work and to enjoy the time with her children.

Zusammenfassung

In dem hier dargestellten Fall einer zweifachen Mutter wurde durch das Auftreten von Anfällen ein Tumor diagnostiziert und chemotherapeutisch behandelt. Eine antikonvulsive Behandlung mit Oxcarbazepin brachte keine Anfallsfreiheit und führte zum Auftreten einer schweren Hyponatriämie. Unter der Kombination von Levetiracetam (LEV) und Carbamazepin (CBZ) sistierten die Anfälle und die Hyponatriämie bildete sich zurück. Wegen der starken Müdigkeit wurde die CBZ-Dosis reduziert und auf eine Monotherapie mit LEV umgestellt. Unter der alleinigen Behandlung mit LEV klang die Müdigkeit ab, und die Patientin kann ihre Alltagsaufgaben wieder bewältigen.
