Abstract
Etiologic factors have a significant impact on treatment and prognosis in pediatric epilepsy syndromes. Although a large proportion of epileptic conditions in childhood are idiopathic and may be associated with an age-dependent remission, seizures may be a predominant symptom of an underlying neurologic disorder. MRI may demonstrate findings suggestive of neurometabolic or storage disorders and in conjunction with the clinical neurologic picture may give valuable clues for the etiologic diagnosis. We present 2 children with epilepsy, evidence of neuromuscular irritability and bilateral basal ganglia and thalamic calcification due to hypoparathyroidism and pseudohypoparathyroidism. Symptomatic treatment with antiepileptic drugs continued for a number of years. Only once the underlying disorder of calcium metabolism was diagnosed and treated did all the symptoms of both patients remit. Basal ganglia calcifications should not be considered idiopathic unless a treatable cause has been excludedespecially in children.
Zusammenfassung
Ätiologische Faktoren sind ausschlaggebend für die Behandlung und Prognose kindlicher Epilepsiesyndrome. Wir beschreiben zwei Kinder die über mehrere Jahre mit Antiepileptika wegen rezidivierender Anfälle behandelt wurden. Eine erweiterte Diagnostik demonstrierte bilaterale Basalganglienverkalkungen im MRT und die Labordiagnostik bot Hinweise für einen Hypoparathyreoidismus und Pseudohypoparathyreoidismus bei den beschriebenen Patienten. Erst die kausale Therapie führte zu einer kompletten Remission der Anfälle und assozierten Symptomatik beider Patienten. Bilateral Basalganglienverkalkungen sollten nicht als idiopathisch betrachtet werden bevor symptomatische Ursachen ausgeschlossen wurden. Dies gilt insbesondere für das Kindesalter.
