Zusammenfassung

Definition

Akute Atemnot gehört zu den häufigsten pädiatrischen Notfallsituationen und ist meistens obstruktiver Natur. In der klinischen Einordnung ist hierbei differenzialdiagnostisch bei exspiratorischem Stridor und Giemen der akute Asthmaanfall oder Status asthmaticus die häufigste Differenzialdiagnose.

Risikofaktoren

Der häufigste Risikofaktor für einen fatalen Ausgang einer Asthma-Exazerbation stellt eine verzögerte Behandlung dar, weshalb der frühzeitigen und konsequenten präklinischen Behandlung höchste Priorität zukommt.

Therapie

Hauptsäulen der Therapie sind die inhalative Verabreichung von β-2-Sympathomimetika sowie systemische Steroide. Bei mangelndem Ansprechen stehen Suprarenin inhalativ, β-2-Sympathomimetika intravenös oder subkutan, Adrenalin i.m. oder i.v., Theophyllin oder Magnesium zur Verfügung.

Besondere Aspekte

Im Falle einer invasiven Beatmung muss ggf. wegen Überblähung der Lungen eine manuelle Unterstützung der Exspiration durch Kompression des Thorax erfolgen.

Dyspnea is one of the most common pediatric emergencies and is mostly of an obstructive origin. In cases of expiratory stridor and wheezing, acute asthma and status asthmaticus are the most common causes.

Risk

Correct diagnosis and early preclinical therapy are therefore necessary to avoid life-threatening situations.

Therapy

The main therapeutic measures are inhalation with beta sympathomimetics and systemic steroids. In cases of deterioration or lack of improvement epinephrine per inhalation, intravenous or subcutaneous beta sympathomimetics, i.m. or i.v. adrenaline, theophylline or magnesium are therapeutic options.

Spezial points

If the patient is intubated manual compression of the thorax during expiration is often necessary for adequate ventilation.
